CHICAGO, June 24, 2025 – Nationwide class action law firm FeganScott has announced an investigation into the Episource data breach impacting over 5.4 million people. Episource is a healthcare technology company that offers software services and risk adjustment platforms widely used by health plans and providers. According to a report provided to the Department of Health & Human Services on June 6, 2025, more than 5 million people may have had personally identifying and protected health information stolen by an unauthorized third party, including:
- Health insurance information, such as health insurance plans and policies, Member/Group ID numbers, Medicaid-Medicare-government payor ID numbers
- Medical records, medical record numbers, doctors, diagnoses, medicines, test results, images, care, and treatment
- Other personal data including Social Security numbers and birthdates
In a notice provided by the company, Episource says that the data breach occurred between January 27, 2025, and February 6, 2025, during which time a cybercriminal was able to access and copy data from Episource’s computer systems. After removing the cybercriminal from their computer systems, Episource waited until April 23, 2025 to begin notifying customers about which individuals and specific data may have been involved.
If you or someone you know has used Episource’s platforms or services in the past, or have received written notice from Episource or a healthcare provider about your information being part of the breach, please contact FeganScott to learn more about your rights:
